Giant Piranha: Find three Leeches lying on the ground around the Forest, return them to the woman by the lake, and take the Leech Bait she gives you. Equip it, cast, and defeat the Giant Piranha — kill the smaller piranhas first — then return the boss drop to her for the Island 3 coordinates.
